About Case Manager / Care Coordinator roles

Case management in mental health involves coordinating a person's care across multiple services — clinical, social, housing, financial, and community. Case managers conduct ongoing assessments, facilitate care planning, liaise with psychiatrists and allied health, and support people to achieve their recovery goals. The role requires strong organisational skills, clinical judgment, and the ability to work with complexity.

In the public sector, case managers work within community mental health teams (CMHTs) and are often the primary point of contact for people with severe and persistent mental illness. In the NDIS, Support Coordinators and Specialist Support Coordinators perform a similar coordination function for participants with psychosocial disability.

Case management roles are filled by a range of professionals — nurses, social workers, occupational therapists, and psychologists all work in case management, with the professional background informing the clinical lens brought to the role.
